How we handle your information

What this website collects

When you use this site, we may collect:

• Contact details you enter into a form — your name, phone number, email address, and where relevant a service address and ZIP code.
• Booking preferences — the service you are interested in, preferred dates and times, and any notes you choose to add.
• Event and collaboration details, if you submit an inquiry through that form.
• Basic technical information your browser sends, such as device type and pages visited, used to keep the site working and understand which pages are useful.

What we ask you not to send

Please do not enter detailed medical information, test results, or diagnoses into any form on this website, or into the on-site concierge. Those channels are not a secure medical record system.

Our team will collect anything clinical from you directly, through a secure intake process, once your appointment is confirmed.

Medical records are handled separately

Protected health information created during your care — your intake, laboratory results, provider notes, and prescriptions — is held in our clinical systems, not on this website, and is governed by HIPAA and California medical privacy law rather than by this page.

We maintain a separate Notice of Privacy Practices describing how that information is used and disclosed and what rights you have over it. Ask for a copy at reception or call (818) 906-4404.

How we use what you send

We use the information from this website to:

• Contact you about the appointment, visit, or inquiry you submitted.
• Confirm eligibility and scheduling, including any clearance or laboratory requirement.
• Answer questions you have asked us.
• Keep our services and this website running properly and securely.

Who we share it with

We do not sell your personal information, and we do not share it with advertisers.

We share it only with service providers who help us operate — for example our scheduling or booking platform, our email provider, and our website host — and only so they can perform that work for us. We share it where the law requires, or where it is necessary to protect someone's safety.

How long we keep it

Website inquiries are kept for as long as needed to respond and to maintain a record of the request. Medical records are retained for the period required by California law and professional obligations, which is longer.

Your California privacy rights

If you are a California resident, you have the right to know what personal information we collect about you, to request a copy of it, to ask us to delete it, to ask us to correct it, and not to be discriminated against for exercising those rights. We do not sell or share personal information for cross-context behavioural advertising.

To make a request, call (818) 906-4404 or ask at reception. We may need to verify your identity before we act on it. Note that medical records subject to HIPAA follow a separate access process, which we will explain when you ask.

Cookies and analytics

This website uses only what it needs to function. If we add analytics or advertising tools in future, we will update this policy before doing so and describe what they collect.

Children

This website is intended for adults. We do not knowingly collect personal information through it from children under 13.

Changes to this policy

If we change this policy we will update the date at the bottom of this page. Material changes will be highlighted on the site.
